Licensed Practical Nurse!
Looking for a compassionate Licensed Practical Nurse with experience in care of patients with mental health and substance use disorders. Skilled in medication administration, documentation and substance use disorders. Committed to providing a therapeutic environment, meticulous documentation, and collaborative care in our Medicated Assisted Treatment Clinic.
What you will do.
Deliver health education to individuals with severe mental illness and substance use disorders.
Provide medication training focusing on compliance, benefits, and managing side effects.
Lead wellness initiatives for metabolic disorder prevention and physical health management.
Support Complex needs of individuals facing co-occurring mental and physical health conditions.
Coordinate patient care seamlessly alongside external medical and treatment providers.
MAT Specific Job Duties:
Consistently takes a proactive role in reviewing, assessing, and referring patients to qualifying SOR funded services.
Maintains 80% compliance to obtaining GPRA surveys at intake, 6-month follow up, discharge, 3-month post discharge, and 6-month post discharge.
Maintain productivity level of 65% of hours worked per month.

Additional Requirements:
Successful pre-employment medical screening (drug test & TB)
Level 2 Background Screening (DCF & AHCA)
Completion of one week orientation
Requires an L.P.N. certificate in nursing and a clear/active Florida LPN license.
One year of experience in health, human services field is preferred.
Must be able to attain competency with company electronic health record and use it effectively within required timeframes.
Computer skills in Windows operating systems required.
Bilingual a plus. If applicable, must possess a valid Florida driver’s license, current auto insurance, a reliable vehicle and a good driving history.
