Care Coordinator:
Looking for a compassionate Care Coordinator with experience in multi-campus care for patients with mental health and addiction diagnoses. Skilled in documentation, communication, collaboration with treatment providers and creating a therapeutic environment for our patients.
What you will do.
Successfully work with patients in an effort to link them to needed resources, coordinate such resources as necessary, and monitor implementation.
Resources may include: Recovery oriented support systems, mental health resources, substance use services and treatment, housing, financial, vocational, connection to social activities, and other resources to promote independence and enhance daily living.
Identify and develop listings of community-based resources in order to offer patients a wider array of choices.
Consults with other treatment team providers on a regular basis and communicate needs of the patients.

Additional Requirements:
Successful pre-employment medical screening (drug test & TB)
Level 2 Background screening (DCF & AHCA)
Completion of orientation and in unit training
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
Computer skills in Windows operating systems required.
Must be able to attain competence with company electronic health record.
