Job Responsibilities:
Full-time or part-time positions available!
This is a hybrid work position, with time primarily spent in a home/office environment. Some travel in the community is required. Office requirements are minimal and limited, but can be adapted based on your work preference. This position has a flexible schedule Monday – Friday, with no on-call hours or weekend hours. Ability to work independently and communicate effectively is required.
The referral coordinator receives and processes all incoming referrals to the community services programs. The Referral Coordinator will complete intake screening process for individuals who may be diagnosed with a mental health, substance use, and/or co-occurring disorder who may have current or previous legal involvement, high utilization of services, pregnant woman or women with dependent children.
Responsibilities include screening individuals by phone or in person to determine patient needs, providing linkage to community resources, maintaining and tracking database of individuals referred until transitioned to other services. The referral coordinator may travel to area Baker Act facilities to conduct face to face assessments with potential referrals. The referral coordinator communicates with other programs within the agency to facilitate smooth transitions between programs.
We offer mileage reimbursement for all community travel done on behalf of a patient. Company laptop and iPhone are provided.

Job Qualifications:
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and one year of full time or equivalent experience working with adults experiencing serious mental illness AND OR co-occurring. Must possess a valid Florida driver’s license and the appropriate level of automobile insurance coverage. Must have a reliable vehicle. Must be able to attain competency with company electronic health record and use it effectively within the required timeframes.
Training Requirements: Completed or agrees to complete AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services. Must be certified within 6 months with Florida Certification Board as a Certified Behavioral Health Case Manager.
