Job Description/Essential Functions:
Clarity Healthcare is seeking a Licensed Clinical Social Worker (LCSW/LPC).  

Job Summary/Essential Job Functions: Responsible for working in a primary care setting facilitating screenings, brief interventions, and referral to Substance Use Treatment (SBIRT) as well as serve as a resource for Medication Assisted Treatment.  Care Coordination between the primary care team and substance use disorder treatment team, when treatment referral is made, is a key aspect of this job.  Knowledge & practice in the treatment of co-occurring disorders and trauma informed care is preferred.  SBIRT Training will be provided upon hire.


Experience and Education Qualifications:

  • Holds licensure with the applicable board of professional registration for the state that they are providing services in for licensure as a professional counselor or social worker.
    • Licensed Clinical Social Worker (LCSW), OR
    • Licensed Professional Counselor (LPC)
